Mayflower Municipal Health Group (MMHG) offers all benefit eligible new hires guaranteed coverage* for basic life insurance and voluntary term life insurance.
New hires have 31 days from date of hire to enroll with no medical questions.
*Guaranteed coverage means no medical questions or exams are required. Only those previously declined coverage or those requesting over the guaranteed amount would need to provide evidence of insurability.

What is Guaranteed Coverage

  • $10,000 basic life and AD&D insurance – cost is shared between employee and employer
  • Employees under the age of 60 can elect up to $150,000 in voluntary life and AD&D starting for as little as $4.00 per week
  • Coverage available up to $500,000 (to a maximum of 5x salary) with evidence of insurability
  • Guaranteed coverage for dependent children up to age 26
  • Additional coverage for spouse available up to 50% of employee coverage to a maximum of $100,000 with evidence of insurability
  • Issue age pricing – your age bracket is fixed at your initial entry into the voluntary plan
  • Convenient payroll deduction
